Can anyone suggest some references for first principles derivation of power and sample size formulae as used in e.g. Statistical Power Analysis for the Behavioural Sciences by Jacob Cohen.

I am not sure if this is exactly what you want, but there is a useful website that offers free G*power software for making such calculations, with suggested literature: http://www.psycho.uni-duesseldorf.de/abteilungen/aap/gpower3/literature.
